Membership Costs

Prices are figured at a yearly rate.

  • First family member: $500
  • Each additional immediate family member (up to four): $350
  • Each additional immediate family member after four: $300

* Please note that to have multiple family members covered on this plan, all family members must live in the same household and be added at the time of payment.

PaymentS
  • Enrollment fees must be paid in full to receive discounts.
  • All payments are due at the time of service to receive the discount. Any services that are not paid in full at the time of service will not be billed at our regular fee rate.
  • All payments are nonrefundable.
  • No refunds will be given if a member and/or spouse or children do not use the membership, relocate or obtain dental insurance.
  • 12-month term effective from sign up date to renewal date.
Terms and Limitations of the Membership

This membership may NOT be used in conjunction with any other insurance coverage. This agreement will renew at the end of its term automatically unless otherwise specified by the member. All patients are subject to Elite Smiles’ policies.

 

  • This is an In-Office Membership and is NOT dental insurance.
  • Patient portion is due at the time of service for any work received.
  • Lab/material fees, sedation, and dental products are excluded from this membership program.
  • This membership is only available at Elite Smiles office. Therefore, if you are referred to a specialist, they will NOT accept this membership service.
  • Family members cannot be added at any time during the year. This will ensure everyone has the same start and end date of the household membership.
  • This membership is non-transferable, meaning family members cannot be substituted in for another family member.
  • Plans and fees are subject to change yearly.
  • Elite Smiles reserves the right to discontinue this membership for any member at any time.
