Consultation Requests

Submitting a form, sending a message, or requesting a consultation does not guarantee an appointment time until our office confirms it. If you need to cancel or reschedule, please contact our office as soon as possible.

SMS/Text Messaging Terms

Message frequency may vary based on your inquiry, appointment status, and communication with our office. Message and data rates may apply. Consent to receive text messages is not a condition of purchasing any goods or services.

You may reply STOP at any time to opt out of text messages. You may reply HELP for help. After you opt out, we may send one final message confirming that you have been unsubscribed.

Carriers are not liable for delayed or undelivered messages.

No Emergency Use

Do not use website forms or text messaging for dental or medical emergencies. If you are experiencing an emergency, call 911 or seek immediate medical attention.
